Editing "Document" form does not prompt to change the values

Hi Thomas,

I have added fields to the "Document" form such as "Date Created", "Last updated date" and "Type" as shown below but when I tried to edit the form, I am not able to add value to these fields. The fields are showing on the Form Grid but not able to edit them as shown below, it is only displacing the default fields which are "Name" and "Description"

Works for me. Added Doc Name, Doc Notes to the form and they are shown. Running the latest version.
